Case Management

The visiting nurse Case Manager is committed to communicating and working in partnership with patient's physician and insurance representative on an as needed basis. The Case Manager coordinates and manages each level of care and needs with a broad range of comprehensive services. The Patient's health care plan and developments are monitored and reassessed on an ongoing basis to meet patient's goals. Care giving Team Includes

  • Registered and/or Psychiatric Nurses
  • Physical, Occupational and Speech Rehabilitation Therapist
  • Social Worker
  • Home Health Aide
  • Homemaker
  • Nutritionist

Home Health Aides

The Home Health Aide (HHA) can assist patients with daily living activities when abilities may be limited due to illness, injury or age. The Home Health Aide work under the supervision and direction of the patient's nurse. This type of service allows the ill, elderly or disabled patient to remain independent at home.

Services May Include:

  • Assistance with personal care and bathing
  • Assistance with ambulation and mobility
  • Assistance with meal planning and preparation
  • Medication reminders
  • Light housekeeping
  • Laundry

All Home Health Aides are trained and supervised on an ongoing basis. Home Health Aides are assigned according to physician's orders and closely directed by the Case Manager.

Rehabilitation Therapies

The patient can receive professional rehabilitation and counseling services in the privacy and comfort of home. Support is available with managing Illness, injury, and loss of limb functions, impaired speech and difficulty with daily activities.

Rehabilitation outcomes are restoration, improvement, or maintenance of the patient's optimal level of functioning, self- care, self-responsibility, independence, and quality of life.

Physical Therapists:

  • Assist with regaining use of limbs
  • Patients recuperating from strokes, bone fractures, joint replacement, and other incapacitating conditions.
  • Arrange for physical aid equipment and teaching instructions for safety and proper techniques for use in daily
  • living activities.

Occupational Therapists:

  • Provide care for patients with upper extremity limitations, illness, or injury that interferes with ability to perform
  • Daily living activities or homemaking abilities by providing functional equipment and training.
  • Help patient regain lost skills by teaching new ways of performing daily tasks.
  • Equipment and home surrounding modifications for easy adaptability.

Speech Therapists:

  • Provide therapy to improve or restore speech.
  • Perform speech and language evaluation.
  • Teach speech/language patterns and controls to patients recovering from conditions affecting speech a language functions.
  • Teach alternative ways of communication.
  • Treat swallowing and related muscle disorders.

Social Services

Medical Social Worker

  • Provides counseling during stressful times of illness or injury.
  • Assist with identifying community service referrals and benefit programs.

Home IV Infusion Program

Administer intravenous fluids and medications to patients at home with the same high quality and safety found in hospital facilities. HMS program adheres to strict procedures and standards that have been reviewed and approved by the State of Texas and by the Joint Commission of Accreditation of Healthcare Organizations.

Nutrition Couseling Service

Provides interdisciplinary nutrition care planning. After initial assessment, a nutrition screening and interdisciplinary assessment is used to define the patient's nutrition status. Then based on integrated information, a specific nutrition intake is recommended and developed to meet individual needs.

Service includes ongoing monitoring of progress and assistance with obtaining goals:

  • Assist with understanding guidelines of a medically prescribed diet.
  • Diet behavior modification.
  • Provide informational resources regarding condition.
  • Provide useful shopping hints.
  • Provide menu-planning techniques.

Attendant Care Service

The Attendant program is intended to help patient or family member in performing household activities in order to maintain a safe and clean environment.

  • Prepare and serve meals and assist with feeding if necessary.
  • Change bed linen and perform light housekeeping duties.
  • Do shopping for groceries or prescription drug needs.
  • Escort to specific appointments via public transportation.
  • Provide service by the hour.

Attendant Supervisor visits home and conducts a needs assessment prior to placing an Attendant. Attendants have ongoing supervision and orientation.
