The visiting nurse Case Manager is committed to communicating and working in partnership with patient's physician and insurance representative on an as needed basis. The Case Manager coordinates and manages each level of care and needs with a broad range of comprehensive services. The Patient's health care plan and developments are monitored and reassessed on an ongoing basis to meet patient's goals. Care giving Team Includes
The Home Health Aide (HHA) can assist patients with daily living activities when abilities may be limited due to illness, injury or age. The Home Health Aide work under the supervision and direction of the patient's nurse. This type of service allows the ill, elderly or disabled patient to remain independent at home.
All Home Health Aides are trained and supervised on an ongoing basis. Home Health Aides are assigned according to physician's orders and closely directed by the Case Manager.
The patient can receive professional rehabilitation and counseling services in the privacy and comfort of home. Support is available with managing Illness, injury, and loss of limb functions, impaired speech and difficulty with daily activities.
Rehabilitation outcomes are restoration, improvement, or maintenance of the patient's optimal level of functioning, self- care, self-responsibility, independence, and quality of life.
Administer intravenous fluids and medications to patients at home with the same high quality and safety found in hospital facilities. HMS program adheres to strict procedures and standards that have been reviewed and approved by the State of Texas and by the Joint Commission of Accreditation of Healthcare Organizations.
Provides interdisciplinary nutrition care planning. After initial assessment, a nutrition screening and interdisciplinary assessment is used to define the patient's nutrition status. Then based on integrated information, a specific nutrition intake is recommended and developed to meet individual needs.
The Attendant program is intended to help patient or family member in performing household activities in order to maintain a safe and clean environment.
Attendant Supervisor visits home and conducts a needs assessment prior to placing an Attendant. Attendants have ongoing supervision and orientation.
HMS internal Billing Office processes all billing correctly, timely, and follows up to ensure optimum financial success.
